The Biomechanical and Clinical Study of Proximal Fracture of Tibia Treated with Intramedullary Interlocking Nail

Abstract
Objective To clear the biomechanical properties of three fixation instruments in tibia proximal fracture,and 28 cases of patients treated with intramedullary interlocking nail were followed up.Methods 32 tibia specimens were divided into four groups at random,one group was normal contrast,the other groups were made into oblique fracture modals in Ⅱarea of the tibia,and were fixed by Grosse-kempf nails,L-dynamic compression plates(L-DCP) and unilateral external fixations.Bone density of proximal tibia were examined by quantitative compute tomography.The compression test,three-bending test and stress-strain test were conducted on each specimen.All the patients treated with intramedullary interlocking nail were followed up.Results(1) Bone density of individual test groups did not differ significantly from each other.(2) The rigidity of anticompression shows G-K nail is the strongest,and anti-bending test shows L-DCP is the strongest,there is significant difference in all groups.External fixation is the least.(3) The varies of stain in G-K nail is the weakest compared to L-DCP and external fixation.(4)Clinical study were evaluated according to the criterion of Joher-Wruhs,the excellent and good rates are 78.1%.Conclusion Interlocking intramedullary nail can achieve the firm fixation and stress shielding is the least in the proximal fracture of tibia.
