The surgical techniques and clinical effects of minimally invasive plate osteosynthesis for the treatment of humeral shaft fractures

Abstract
Objective To discuss the surgical techniques and clinical effects of minimally invasive plate osteosynthesis (MIPO) for the treatment of humeral shaft fractures. Methods Eighteen patients with humeral shaft fractures were treated by MIPO technique from June 2012 to June 2015. There were 13 males and 5 females with a mean age of 43.2 years (range, 17 to 66 years). According to AO/ASIF fracture classification, 3 were type 12A fractures, 6 were type 12B and 9 were type 12C. The postoperative function recovery was evaluated by disabilities of the arm, shoulder and hand (DASH) score. Results All the cases were follow-up for 6 to 18 months (mean, 12.5 months). 17 fractures were successfully cured except for one nonunion. One case had superficial incision fat liquefaction that was healed by dressing change. There were no serious complications. With regard to the range of motion of the shoulder, the mean flexion was 165° (range, 140° to 170°). The mean extension was 36° (range, 20° to 40°). The mean abduction was 87° (range, 80° to 90°). For the elbow, the mean flexion was 142° (range, 130° to 150°), and the mean extension was -2° (range, -8° to 0°). At the last follow-up, the mean DASH score was 10.6 (range, 5 to 38). Conclusion MIPO technique is recommendable for humeral shaft fractures, especially for the comminuted fractures with the advantages of microlesion, less complications and good functional recovery. Solid anatomical knowledge and skillful surgical techniques are the key for successful treatment of humeral shaft fractures by MIPO.
