Abstract
Aim of the project: to improve the quality of anatomy scans which is an essential screening examination in pregnancy. Methods: The project involved the following steps: 1. Initial baseline audit of anatomy scans from November 2018 to January 2019 against the international standards (ISUOG and Fetal Anomaly Screening Program, FASP, UK) to identify the gaps in the knowledge and skills. 2. Mandatory education of all sonographers on essential principles for standard performance of anatomy scan based on ISUOG and FASP criteria and focused cardiac examination sessions. 3. Prospective audit of all anatomy scans performed at 18- 22 weeks from April 2019 to November 2022 by Fetal Medicine consultants to evaluate the quality and completion of anatomy scans. 4. Regular evaluation of the performance every 3 months through data analysis. 5. Regular one-to-one feedback and performance reviews with sonographers addressing the gaps. Fetal heart anatomy was the most incomplete structure – 510 cases (57%) followed by the spine 216 cases (24%) and face 211 cases (23%). Multiple structures were incomplete in 293 cases (33%). The completion rate of anatomy scan steadily improved from 50% to 70% by 6 months and up to 90% by 9 months through the project This rate was maintained in all the quarters of 2021 and 2022. Individual sonographer performance improved to above 80%. The upskilling project was embraced by the whole team well. By creating a positive environment for learning and development, we were able to augment the existing knowledge and skills of the sonographers and deliver the best care to our patients. We hope to maintain this achievement through regular quality checks and continuing education.
