AI Metaverse in Healthcare: Opportunities, Challenges, and Future Directions

Abstract

The convergence of Artificial Intelligence (AI) with Virtual Reality (VR), Augmented Reality (AR), Mixed Reality (MR), and digital twins is shaping the emergence of an AI-enabled healthcare metaverse. Unlike conventional telemedicine, the metaverse enables persistent, collaborative, and intelligent three-dimensional (3D) environments for medical education, clinical care, and patient engagement. Evidence shows that VR can reduce pain and anxiety by 20–35%, accelerate training by <tex xmlns:mml="http://www.w3.org/1998/Math/MathML" xmlns:xlink="http://www.w3.org/1999/xlink">$30-40\%$</tex>, and improve rehabilitation outcomes. In India, representative Randomized Controlled Trials (RCTs) confirm VR's value in pediatric dentistry and labor analgesia, while pilot deployments such as Apollo Hospitals' immersive counseling programs and MediSim VR nursing labs illustrate early adoption. Policy frameworks including the Ayushman Bharat Digital Mission (ABDM) and the Digital Personal Data Protection (DPDP) Act 2023 provide enabling infrastructure and governance. This review proposes a roadmap emphasizing multi-center clinical validation, low-cost Extended Reality (XR) innovation, Federated Learning (FL) for privacy, Edge Artificial Intelligence (Edge-AI) for real-time deployment, and ethical inclusivity in avatars, language, and digital twin design. The AI-metaverse has the potential to transform healthcare delivery if pursued with rigor, equity, and interdisciplinary collaboration.
